Assessment surveys

Please complete the survey your clinician has asked you to complete.

When prompted, enter the survey password provided by your clinician.

Adolescent & young adult assessment

Ages 12–25 · Extended gender surveys

Allow approximately 30–45 minutes to complete your survey. A scored report will be emailed to your clinician after submission.

For a parent completing the assessment survey about their child or young adult aged 12–25.

For people aged 12–25 completing their own gender assessment survey.

For adults assigned male at birth (AMAB). Complete this survey if your clinician has requested it.
